Community Care Hub Strategies for DEI

I would like to know what strategies peers are using to improve DEI in their network. In addition to demographics for their community are they collecting and using demographics for the population served in their organization or their network? And if so how are they using organizational/network data to track outcomes and measure progress toward DEI?

    Hi Fran, We are forming a DEI Committee and bringing in a DEI Consultant to help inform this committee's charter, goals, and responsibilities.
