NLC and HELC Joint ECHO Session 2/2/24
The second session of the NLC-HELC Joint ECHO series reviewed the topics from the first session, which featured Accountable Care Organizations, referral processes options, and priority populations for community health integration (CHI), principal illness navigation (PIN), principal illness navigation - peer supports (PIN-PS). The session also included a case study on Clarence Blackman, an elderly man who called 911 for food access, and how a CBO can use CHI to assist him and be reimbursed for services.
