The first session of the NLC-HELC ECHO series focused on multiple health equity and social care topics, including an overview of accountable care organizations and tools for identifying priority populations for community health integration (CHI) and principal illness navigation (PIN). The session also included a case study focusing on processes for identifying eligible patients and consent requirements and documentation.
