Description: This Care Transitions Peer Hour will provide an overview of the path that the SARCOA Area Agency on Aging took to build and continue their Hospital to Home transitions program in Alabama. They will share information about how they built their program, including how they encouraged and leveraged collaboration between the AAA, the State Unit on Aging, and the Medicaid agency with the goal of expanding access to Medicaid Waiver and other program services during the pandemic.
