Pre-Learning Session Overview: 

The goal of the Pre-Learning Session was to provide an introduction and overview for participants of both the NLC and HELC to the Community Health Integration (CHI), Principal Illness Navigation (PIN), and Principal Illness Navigation-Peer Support (PIN-PS) HCPCS codes that are were approved as part of the CY 2024 Physician Fee Schedule final rules. This session  provides details about the potential role of CBOs in integrating with healthcare providers to deliver these services. Key implementation details reviewed include establishing priority populations for these services, implementation steps, and evaluation of the impact of interventions deployed.
