Description: Using data from individuals supported by AAAs in the Medicaid waiver program, the Indiana Department of Aging developed a list of high-risk individuals living at home using criteria such as need for dialysis, use of oxygen, trach care. AAAs conducted virtual wellness checks for these individuals, looking for changes in health status, caregiver status, food, and medication supply.  They also discussed emergency back-up plans, secured the names and contact information for caregivers, and made sure individuals and family had contact information for the AAA.
