Region IV Area Agency on Aging and Corewell Health South have partnered to address health-related social needs (HRSNs), with the goal of improving health and driving care in the primary care setting while reducing healthcare costs. Through the interagency care team (ICT) model, Region IV AAA and Corewell Health have successfully resolved 91% of patient barriers while reducing the cost of care by 55%. This case study outlines the ICT model, its impact within the partnership, lessons learned, and keys to the model’s sustainability.
