Looking for CCH resources

Hello, I'm looking for a resource or sample of the following:

1. Written roles and responsibilities for hub administration

2. Evidence/Literature about caregiver services as part of a network service line

3. CCH budget template (for administration, not necessarily for services)

I did look through our resource tab and didn't find anything related. TIA

    Deanna-The Partnership to Align Social Care is working on a roadmap for CCH functions. The attached slides, beginning with 13, may help but once the roadmap is completed I think that will have all info you need about hub administration

    Hi Deana, ACL published evidence summaries for  a number of common CCH services, including caregiver services here: https://acl.gov/programs/strengthening-aging-and-disability-networks/improving-business-practices under "Social Care Services Evidence Summaries". The ADBI also has an Evidence Bank which may have additional evidence on caregiver services.

    Hi Deanna,

    WNYICC has a contract to provide Caregiver Support Program.

    Program Elements:  

    • Member and Caregiver are assigned Health Coach
    • Health Coach meets at member's home to conduct assessment.
    • Coach and Caregiver& Member review Person-Centered Care plan
    • Coach works with Caregiver& Member up to 6 months to:
      • Help set and complete goals,
      • Coordinate and link to services and resources, and 
      • Enroll into an evidence-based workshop (ie CDSMP, CPSMP, Powerful tools for caregivers)
