Call for Innovations

The No Wrong Door Resource Center wants to hear about the innovative ways you are supporting people during transitions from hospital to home. How do you assess and address individuals’ needs at home to improve independence, safety, and health? Have you found a way to creatively tackle challenges in rural areas? Do you use technology to enable transitions?
