Pathways Hub / CCH alignment?

Is anyone else exploring or attempting to align with CCH model with a Pathways Hub? If so,  how are you layering the two approaches? Pathways seems to focus on navigating individuals to resources, while the CCH model allows for navigation and a broader focus on resources to which individuals are connected.

    ACL has met with leadership from the Pathways Community Hub Institute to think through opportunities for partnership/alignment. We'd like to develop a joint communication and send to National Learning Community participants and Pathways HUBS in overlapping geographic areas to highlight how the respective Community Care Hubs and Pathways HUBS can work together. I'd appreciate hearing about any successes/challenges that others have experienced in this area to inform that communication, as well as our ongoing engagement with Pathways - thanks!
