The “Opening Every Door: Marketing State No Wrong Door Systems to Expand Access” webinar on Wednesday, October 14 from 3:00-4:00pm ET will highlight practical approaches to increasing visibility, expanding reach, and connecting more people to the services and supports available in their communities. Join ACL’s No Wrong Door Resource Center for this discussion on strengthening awareness, expanding access, and ensuring that every door can be the right door for individuals seeking information, assistance, and support.

The intended audience for this webinar is No Wrong Door state leads (i.e., No Wrong Door/Aging and Disability Resource Center Directors, Coordinators, and Managers; leaders in aging, disability, and Medicaid; and cross-agency leadership).

Care Transitions ECHO Series: Building Partnerships to Support Health-Related Social Needs – Curriculum Guide

Looking for resources to strengthen care transitions and better connect individuals with community-based supports?

The Care Transitions ECHO Series: Curriculum and Event Guide brings together lessons, tools, presentations, and supporting resources from a series focused on addressing health-related social needs (HRSNs) throughout the transition from hospital to home.

The curriculum walks through key components of effective care transitions, including screening, clinical and community-based organization (CBO) communication and handoffs, CBO engagement, post-discharge supports, Medicare billing opportunities, and strategies for sustaining care transition activities. The guide includes eight sessions, each featuring a brief presentation from subject matter experts along with supplemental resources from organizations such as the Centers for Medicare & Medicaid Services (CMS), Agency for Healthcare Research and Quality (AHRQ), The Joint Commission, and many others.

Seeking Nominations for Family Caregiving-Related Advisory Councils

  • The RAISE Family Caregiving Advisory Council will support the implementation of the update to the National Strategy to Support Family Caregivers that is slated to be released later this year. Implementation efforts will include new developments, challenges, opportunities, and solutions to better recognize and support family caregivers, along with the recommendations to build upon the accomplishments of the National Family Caregiving Strategy in future reports. Up to 15 non-federal voting members will be appointed for three-year terms. Those selected to serve on the Advisory Council will reflect the wide range of experiences of family caregivers and care recipients. To be considered for selection, nominations must include all requested information and attachments and be submitted electronically to RAISE.mail@acl.hhs.gov by 11:59 p.m. ET on October 14, 2026.   • The Advisory Council to Support Grandparents Raising Grandchildren (SGRG) will support the preparation of updates to the National Strategy to Support Family Caregivers that is slated to be released later this year. Specifically, the Advisory Council will focus on areas pertaining to grandparents and older relative (kinship) caregiver support while also creating and augmenting ACL related tools within the Kinship Caregiving arena. The Advisory Council also serves as the primary panel that informs the work of the Grandfamilies and Kinship Support Network: A National Technical Assistance Center. ACL anticipates selecting up to 10 non-federal members to serve on the Advisory Council. These members will include grandparents, older relative/kinship caregivers, and professionals working on their behalf, with particular emphasis on individuals representing tribal communities and families impacted by the opioid crisis. To be considered for selection, nominations must include all requested information and attachments and be submitted electronically to SGRG.mail@acl.hhs.gov by 11:59 p.m. ET on October 14, 2026.
