Care Transitions Webinar 

ACL’s No Wrong Door Resource Center invites you to participate in an upcoming webinar, "Seamless Transitions: Supporting Patients With Chronic Illness Across Care Settings," on Wednesday, July 16, 2025 from 3:00-4:00 p.m. ET. This webinar will focus on:

• Critical intervention points where community-based organizations can effectively support people with chronic conditions during hospital discharge planning, transition back to home, and post discharge; and
• Long-term supports to improve community tenure and avoid hospital readmissions.

AAAs: Helping You Live Long and Stay Strong

Area Agencies on Aging (AAAs) are critical No Wrong Door access points, with 65% also serving as Aging and Disability Resource Centers (ADRCs). As part of their 50th anniversary, USAging has launched a new public awareness campaign—"Area Agencies on Aging: Helping You Live Long and Stay Strong.” The goal of this campaign is to make the public aware of AAAs as a vital community resource and to educate consumers about the range of important services that AAAs offer to older adults, people with disabilities, and caregivers. There are more than 600 AAAs serving every community across the country ready to help them to live well where most want—at home and in their community. NCIL Conference

Registration for the 2025 Annual Conference on Independent Living is open. The conference is hosted by the National Council on Independent Living (NCIL), the longest-running national cross-disability, grassroots organization run by and for people with disabilities. NCIL represents individuals with disabilities and the organizations that advocate for the human and civil rights of people with disabilities throughout the United States. The theme for this year’s conference is LEVEL UP: Building Tomorrow Together. The conference will take place July 21-24 at the Grand Hyatt in Washington, DC. Learn more and register here.
