PCC Resource Update and Age+Action Conference Session

We have updated the No Wrong Door Person-Centered Counseling (PCC) Defined resource. This product covers the definition, components, and key concepts of PCC. The No Wrong Door Resource Center is presenting two sessions at the National Council on Aging’s Age+Action conference, and one of them will focus on PCC.

  • “A Person-Centered Approach to Health Information and Counseling” on Wednesday, May 27 from 10:45-11:45am in Grand Salon A.
  • “A No Wrong Door Approach to Benefits Enrollment” on Thursday, May 28 from 1:45-2:45pm in Grand Salon K.

Ohio Care Transitions Program

The Council on Aging of Southwestern Ohio, part of the state’s No Wrong Door System, manages an award-winning care transitions program called FastTrack Home. The FastTrack Home program helps older adults safely return home after a hospital or nursing facility stay by providing care coordination and in‑home supports for up to 60 days. Care managers assess eligibility and needs prior to discharge, then continue support through home visits and follow-up phone calls with patients and family caregivers. About 60% of participants do not require additional services after the 60‑day FastTrack Home period, highlighting the program’s effectiveness in supporting long-term independence.

Top service needs addressed through FastTrack Home include:

  1. Home‑delivered meals
  2. Home care services (personal care, laundry delivery, and homemaking)
  3. Home medical equipment
  4. Emergency response devices
  5. Home modifications, including ramps
  6. Transportation

The program is primarily locally supported, with approximately 80% of funding coming from county senior services levies. An updated fact sheet about the program can be found here.


Coordination Between No Wrong Door and Kinship Navigator Programs

While there are a few ADRCs (e.g., GA, OR, VT, WA, WI) that serve as, partner with, or link to Kinship Navigator programs, opportunity exists for ADRCs to partner with local kinship care organizations and child welfare organizations in support of grandparents’ needs for assistance. This report from the Administration for Children and Families explores the significant opportunities that remain for states to improve licensing, definitions, and reach to promote kinship care. Be sure you understand the current landscape of your state’s Kinship Navigator program and incorporate coordination into your No Wrong Door planning.
