Council on Aging of Southwestern Ohio: FastTrack Home Fact Sheet

FastTrack Home provides in-home care services when they are needed the most – as an older adult is leaving the hospital or nursing home. Services such as homemaking, home-delivered meals and medical transportation are in place shortly after a patient returns home and remain in place for up to 60 days to help in recovery and prevent unnecessary hospital readmissions. A care manager also works with the client to ensure they understand new medications, follow doctor’s orders and know what “red flags” to look for during recovery. FastTrack Home is paid for via the senior services tax levies in the counties where it operates and is offered at no cost to the client.
