Here is today’s Monday Mailer for you:

State Innovation: California No Wrong Door Navigation

California launched a pilot program, Cal Community Connect, designed to advance California’s No Wrong Door navigation system by providing older adults, adults with disabilities, and caregivers, streamlined access to long-term services and supports through a community health worker (CHW) navigator.

Through this free program, a trained CHW will provide comprehensive, person-centered navigation services to help older adults and adults with disabilities locate and connect with community services, supports, and resources to promote health and wellbeing. This project builds upon successful system change initiatives driven by California’s Multisector Plan for Aging. Learn more about Cal Community Connect here.

Illinois Multisector Plan for Aging

Illinois released EngAging Illinois: A Multisector Plan for Aging, a comprehensive, person-centered blueprint to support aging well across the state through 2036. Illinois is now the 13th state with a Multisector Plan for Aging.

The plan introduces EngAge Central: The Illinois Aging Resource Center that will:

  • Develop a repository of federal, state, and local benefits searchable by locality.
  • Use a No Wrong Door approach by cross-training employees of state agencies, local partners, helplines, and 211 Illinois, so everyone in the state can connect with a knowledgeable agent who can assess and refer to appropriate programs, services, and benefits.
  • Collect and analyze phone call and website data to determine trends in frequently requested resources and areas of unmet need.
  • Use data to inform awareness campaigns, new services, demonstration projects, and other pilot projects.
