NWDRC at HCBS Conference

ACL staff and the No Wrong Door Resource Center (NWDRC) Technical Assistance team will be at the HCBS Conference in Baltimore, Maryland from August 23-27. Similar to last year, we plan to gather for a No Wrong Door lead meet-up at the exhibit hall on Tuesday, August 25, from 8-9am ET. Introduction to CCHs Video

The Center of Excellence to Align Health and Social Care, part of USAging’s Aging and Disability Business Institute, released a new video, An Introduction to Community Care Hubs. The two-minute animated video highlights the role of community care hubs (CCHs) in advancing person-centered health through coordination between health care and community-based services. This is the first in a three-part video series, with additional videos focused on healthcare providers, health systems, and health plans published in the fall. Community care hubs are valuable partners in a No Wrong Door system. Watch the video here.

Multnomah County Aged, Disabled, and Veterans Services Initiative Connects Individuals Experiencing Homelessness with Stabilizing Supports

Multnomah County Aged, Disabled and Veterans Services (the aging and disability resource center) operates a Homeless Mobile Intake Team (HMIT) that connects individuals experiencing homelessness to Medicaid Long-Term Services and Supports (LTSS) through assessment-driven care transitions. In Multnomah County, more than 25% of individuals experiencing homelessness are over age 55, highlighting the growing intersection of aging and housing instability.

Between November 2022 and 2025, HMIT assessed 637 individuals, with 29% determined eligible for and connected to Medicaid LTSS in-home or in a care facility. The multidisciplinary team provides outreach in shelters, hospitals, recuperative care centers, and community settings to evaluate functional need and Medicaid eligibility. Once eligibility is established, the team supports each person in choosing among licensed care settings including home-based care, an adult foster home, assisted living, residential care, or a nursing facility, which are matched to assessed need, preference, and eligibility determination. Short-term “hoteling” may serve as a stabilization bridge while an eligibility-based placement is arranged. A webinar featuring Multnomah County’s HMIT program is available here. 

Housing instability continues to be a significant challenge to successful care transitions, with 8 out of 9 Enhancing State No Wrong Door Systems for Efficient Access grantees identifying it as a primary in their semi-annual grant report.
