Workflows and partner responsibilities resource?

Hi, could someone share a resource for a workflow visual for networks from the perspective of the client? I believe Healthy Living for ME presented one at last year's USAging Conference that was really well done.

Also, I'm looking for examples of roles and responsibilities of network members and vetting criteria. If anyone can share a template or a resource, I would appreciate it.
