Maryland plans to divert individuals from nursing facilities at the point of hospital discharge through strategic partnerships between the ADRC and local hospitals. They plan to use Medicaid Administrative funds and Money Follows the Person (MFP) funds to begin the program. They hope to sustain the program through local hospital funds which will provide the match for the Medicaid administrative funding. This represents an excellent example of blending and braiding funds to accomplish No Wrong Door (NWD) sustainability.
