Building Toward Better Care Coordination: You Don’t Need Perfect Data Systems to Start

When someone is discharged from a hospital or moves between health care settings, timely information can make a meaningful difference in what happens next. Community-based organizations (CBOs) need access to important information so they can connect people with the services and supports they need as quickly and effectively as possible. Stronger information sharing can help CBOs and health care partners coordinate care, respond to changing needs, and support more successful transitions.

Building this capacity does not require a sophisticated data system from day one. CBOs can start with the tools they already have and build their capabilities over time. The goal is to establish reliable processes for tracking clients, documenting services and outcomes, and securely communicating with healthcare partners.

Here are some ways you might start:

• Organize client information: Establish a consistent way to track who you serve, what services they receive, and what follow-up may be needed. This may start with a well-organized spreadsheet or database.
• Strengthen communication: Identify secure, consistent ways to share relevant information and coordinate with health care partners while protecting client information.
• Track and report outcomes: Develop simple ways to document and report outcomes so you can demonstrate the value of your services to partners and identify opportunities for improvement.
• Build toward more advanced data sharing: As partnerships and organizational capacity grow, consider technologies that can support more efficient information sharing, including admissions, discharge, and transfer (ADT) notifications.

The path to stronger data systems is incremental. Rather than waiting until you have the resources for a perfect system, focus on building a reliable foundation that meets your current needs and can grow as your health partnerships develop. Each step can make it easier to share important information, activate needed services, and support people through care transitions.
