Here is today’s Monday Mailer for you:

Care Transitions Success Story

  • USAging’s Aging and Disability Business Institute released a new publication that spotlights the remarkable achievements of the Allegheny County Area Agency on Aging (AAA) in Pittsburgh, PA, which received recognition for its Community Care Transitions Program (AAA-CCTP). This model emphasizes the vital role that community-based organizations can play in enhancing health outcomes and ensuring continuity of care following hospital discharge. The AAA was the runner-up for The John A. Hartford Foundation 2024 Business Innovation Award.

Caregiver Corner

  • The Rosalyn Carter Institute released a series of Impact Papers on family caregivers that provide distinct perspectives on the realities of caregiving and connect evidence with actionable recommendations for progress. One of the papers is on Rural Caregivers. Rural caregivers face distinct realities that shape how they provide care. Family caregivers in rural America often provide care in isolation, with fewer resources and longer distances to navigate.
