Colorado In-Reach Counseling Program to Empower Nursing Facility Transitions
Colorado’s In-Reach Counseling Program is designed to connect residents living in nursing facilities to the information and resources they need to make informed decisions about their community living options. Through both individual and group counseling sessions, the program supports residents in understanding available community-based services, housing options, and the steps required to transition successfully out of nursing facilities. National I&R Support Center Webinar
The National Information & Referral Support Center is hosting the webinar, “In Their Own Words: Person-Reported Data and the State of LTSS in 2024-25” on Wednesday, July 8, from 3:00-4:00pm ET. This session will present person-reported data from across state long-term service and support systems, offering a ground-level view of quality, access, and unmet needs. Attendees will explore what the data reveals about the lived experiences of LTSS participants, and what it means for policy, practice, and advancing quality service delivery.
