Proactive Vaccine Strategy: Michigan local ADRC partners with multiple partners for in home vaccinations!

In May 2021, the Detroit Area Agency on Aging, the local ADRC, launched a program to vaccinate individuals unable to travel to a vaccine site. They piloted the program with the initial goal to vaccine 400 older adults. They partnered with the Institute for Population Health, Health Centers Detroit Foundation, Medicaid waiver home health agencies, the health department, Wayne Health, and the Michigan Primary Care Association. They called over 4000 individuals to assess interest in the program and have vaccinated 335 older adults to date.

    Georgia's Division of Aging Services (part of the Dept. of Human Services) partnered with the Department of Public Health (DPH) and the Department of Community Health to identify homebound individuals participating in both Medicaid and OAA services to get the vaccine delivered to those individuals at home. 

    After the those individuals were identified, DPH created a publicly available phone number and email address for individuals/caregivers to leave contact information for other individuals who may need the vaccine provided in their homes. Those names are continuing to slowly trickle in, but they do get new referrals each week and coordinate to get those vaccines delivered with the local health department.
