  • Multi-State Information Technology (IT) Learning Collaborative Report: In April 2024, the Administration for Community Living (ACL), with support from EMI Advisors (EMI), launched the Multi-State IT Learning Collaborative for cross-agency state teams to learn from each other about strategies to realize interoperable exchange between health and social care systems. This report summarizes key discussions from the Learning Collaborative that focus on supporting sustainable pathways for integrating health and social care while addressing knowledge and IT infrastructure gaps related to referral systems and electronic health and human services information exchange.   • ACL's Eldercare Locator and USAging have launched the 2024 “Home for the Holidays” campaign. This year’s campaign connects older adults and caregivers with information on how to protect themselves from financial scams and fraud. Older adults are especially vulnerable to the financial scams and fraud that start to increase during this time of year. The campaign features a brochure, Protect Your Wallet: A Guide to Avoiding Financial Scams, which highlights common scams that target older adults, tips for avoiding scams and resources for reporting scams.   • The SCAN Foundation Blog - Bringing the National Plan on Aging to Life: Turning Strategy into Action: The Administration for Community Living’s Interagency Coordinating Committee on Healthy Aging and Age Friendly Communities is developing a National Plan on Aging (NPA), a blueprint to guide and align federal policy and programmatic efforts to advance healthy aging opportunities for all older adults. The SCAN Foundation highlights best practices from states, for NPA development, to ensure the NPA supports and reflects the needs of Americans as they age. Read the blog here.
