Caregiver Corner

• November is National Family Caregivers Month! Read President Biden’s proclamation here.

• With support from ACL, ten states (AK, CA, IN, MO, NC, NH, NV, SC, TN and TX) worked to enhance state leadership of their No Wrong Door (NWD) Systems by strengthening their governance structures to include strong collaboration among aging, disability, Medicaid and others. They also conducted a mapping analysis to identify gaps in access to services and supports for caregivers. The states will use the mapping analyses findings to develop plans to enhance their state access systems. States highlighted their work during a NWD Governance/Caregiver Symposium in August. • In January 2024, ACL invited hospitals, health systems, accountable care organizations, and community-based organizations to a special series of learning events and discussions around collaborative approaches to screening for health-related social needs (HRSN), connecting with community-based social services, and supporting transitions pre- and post-discharge. The goal of the series was to provide tools and content to help organizations leverage community partnerships and assets to address patients’ unmet HRSN.
