Description: The Aging and Disability Resource Center of Jackson County, Wisconsin, takes a proactive approach called “Healthy Measures” to increase vaccine awareness and confidence with the support of the ACL NWD/ADRC grant. They began their Healthy Measures vaccine awareness in October 2022 with a Healthy Measures Bingo Bash for Seniors. Twenty-two seniors participated in an afternoon of ADRC-themed Bingo, featuring Q&A on aging and disability services, vaccines, and public health. All in attendance received a Healthy Measures Tote Bag. The insulated tote bag, which can later be used for grocery shopping, contains a “COVID Kit” with a cold pack, thermometer, pulse oximeter, COVID-19 At Home Test, a face mask, and hand sanitizer. The tote bags also have information on ADRC services, public health services, Community Health Worker contacts, a how-to guide for seniors on protection from fraud by tracking health care, and an Emergency Preparedness Toolkit for persons with physical disabilities. Medical professionals from Jackson County Public Health partnered with the ADRC to answer questions and share information on available vaccines and the Elder Benefit Specialists provided information on insurance coverage for vaccines.

In November 2022, a community health worker visited four congregate meal sites during Social Isolation and Loneliness Awareness Week. They distributed 89 Healthy Measures Tote Bags to seniors participating in Senior Meal Sites and Home Delivered Meals. An additional 20 tote bags were distributed at a Senior Carnival, which was supported by a partnership with Jackson County Interfaith. During this outreach, many seniors took a three-question Social Isolation and Loneliness Screen. When the screen indicated possible need, the community health worker provided additional information on upcoming events, a referral to the ADRC, and other services to promote community participation.
