Nevada: Leveraging No Wrong Door Governance for a Monitoring and Evaluation Framework to Support Continuous Quality Improvement
Nevada's lead No Wrong Door entity, the Aging and Disability Services Division (ADSD), used their No Wrong Door system grant to develop a Monitoring and Evaluation (M&E) Framework for their Nevada Care Connection (NVCC). The M&E Framework focuses on system-level tracking, designed to inform continuous quality improvement and support long-term sustainability. The governing body established by ADSD under the grant, the Nevada Care Connection Collective (NCCC), is the primary body responsible for reviewing progress and guiding course corrections for the NVCC. NCCC meetings align with key planning and budget timelines to allow state and local decision-makers to set statewide objectives, resolve system-level issues, and guide the strategic direction of the NVCC. Participating entities include:
- Aging and Disability Services Division
- Nevada Medicaid
- Division of Welfare and Support Services
- Division of Public and Behavioral Health
- Division of Child and Family Services
- Department of Employment, Training and Rehabilitation
- Office of Data Analytics
- Nevada Department of Veterans Services
- Nevada Department of Transportation
- Nevada Housing Division
- Statewide Independent Living Council
- Nevada Governor's Council on Developmental Disabilities
- Tribal representatives
- Regional and local access partners
- Individuals with lived experience
To strengthen the NVCC, the NCCC will facilitate system-wide monitoring using a set of shared performance indicators aligned with each of the NCCC's strategic goals, outlined in the graphic below.

Performance indicator metrics will be tied to data sources such as Access Nevada referral data, partner reporting, and internal tracking, including measures such as referral volume, referral completion rates, average time from referral to service connection, utilization of person-centered tools, and regional service coverage. Metrics will focus on outcomes such as:
- Number of agencies and partners participating in NCCC meetings (i.e., formal No Wrong Door governance meetings)
- Implementation of referral tracking protocols across access points
- Timelines and follow-through on referrals across agencies
- Availability and use of person-centered counseling tools
- Regional and demographic reach of navigation and access services
- Integration of NVCC goals into state budget submissions and planning cycles
The NCCC will use findings to assess implementation of strategic goals; identify operational barriers or gaps in system alignment; recommend technical assistance, training, or policy adjustments; and inform future funding strategies and sustainability planning. To ensure accountability, the NCCC will document all decisions and system adjustments in an annual Nevada Care Connection Report, which will be shared with Department of Human Services leadership and made available to the public. Additionally, the NCCC will maintain opportunities for public and stakeholder input through targeted outreach, listening sessions for individuals with lived experience, and feedback from partner organizations and boards.
