Description: The CMS Medicare-Medicaid Coordination Office (MMCO), Integrated Care Resource Center issued a Medicaid Directors guidance document outlining how D-SNPs can meet this requirement by contracting with Medicaid to improve care transitions.  The guidance document highlights the impact of CCTP in creating cost savings to the Medicare program and the Mathematica 2017 final report showing statistically significant cost savings data.  The document also references the fact that D-SNPs and Medicaid could use the option of contracting with  AAAs, CILs, and ADRCs to meet this requirement.
