The Virginia Department for Aging and Rehabilitative Services developed this digital flyer for local ADRC sites to encourage proactive vaccine outreach. It provides specific calls to action including considering social determinants of health and solicits information from local entities leading efforts across the Commonwealth of Virginia. As a result they have been able to put together a series of reports highlighting lessons learned and successes and promising practices. This an excellent example of a state embracing the 5 Steps to Proactive Vaccine Outreach, especially in identifying barriers and demonstrating success.
