Health & Social Care
Health care entities and community-based organizations (CBOs) are increasingly recognizing the importance of partnerships to fully address a person’s overall well-being. This includes partnerships to address health-related social needs (HRSNs) such as housing instability, food insecurity, and lack of transportation. Community care hubs (CCHs) and their networks of CBO providers support the NWD goal to increase access to services for individuals with unmet needs. CCHs centralize administrative functions and operational infrastructure to enable effective, sustainable partnerships between CBOs and health care.
Housing & Services Partnership Accelerator
The Housing and Services Partnership Accelerator (HSPA) is a technical assistance opportunity for states implementing housing-related services and supports for older adults and people with disabilities who are homeless, institutionalized, or at risk of either. The Accelerator is headed by the Housing & Services Resource Center (HSRC) -- a collaboration between federal agencies seeking to streamline access to housing, health, and social care services and increase efficiency across sectors.
