PA Link: Person-Centered Counseling Operational Guide and Assessment Form
The Pennsylvania Link to Aging and Disability Resource Office (ADRO) designed the Person-Centered Counseling (PCC) program in order to better guide Pennsylvanians on the wide range of public and private resources available to them. The goals of the program include reducing gaps in service, empowering individuals to take action regarding their long-term care and streamlining eligibility. The Operational Guide outlines Pennsylvania’s Department of Aging (PDA) approach to implement PCC state-wide. The PCC Assessment Form is used by counselors to document the PCC process and ensure that consumers receive a consistent and similar experience across the state.
